This video documents the first use of tele-robotic manipulation using the Da Vinci surgical manipulator on the treatment of brachial Plexus injury, on Latin America. On this clinical case the robot is used on the place of the microscope. Conventional approach was done to explore the plexus and prepare the reconstruction, using sural nerve graft. When the microsurgical steps were supposed to start the robot was docked behind the patient and the micro nerve anastomosis using 9-0 suture was performed, under tele-robotic manipulation. On this case fibrin glue and wrapping using colagen nerve conduits (Neuragen-Integra) were added to the technique to prevent adhesions and optimize the nerve axonal flow. The benefits of tele-robotic manipulation on microsurgical procedures include: 1- great visual magnification with HD resolution and 3D images, 2- best ergonomic situation for the surgeon, in any anatomic area, 3- physiological tremor filter 4- amplification of surgeon's dexterity and functional range of motion The robotic system improves not only how good we can see, as the microscope, but also how good we can do. For more information about the use of robot on microsurgery: www.rash-society.org www.roboticmicrosurgeons.org diclosures: none of the surgeons involved on the surgery received any financial benefits from the companies Intuitive Surgical or Integra.

The video i shot in the ot today. If you watch closely, you will see that Dr Dogra seating near the robotic console is remotely manipulating the instruments placed in the patient's body. Advantages: he is getting a 3D view in the console 's screen and because he is sitiing, he doesnt get tired ; there are also less complications in the procedure. But the system costs over $2.5 million. I hope that one day I will operate a similar console completely on my own !

For more information visit: www.pluralpublishing.com Cancer surgery of the mouth and throat can involve major incision wounds, tracheotomy, or necessitate the breaking of bones. Recognizing the benefits and success of robotic surgery in other areas of medicine, the authors present its use in head and neck surgery, resulting in less surgery, less deformity, and faster rehabilitation. In this new book, the authors describe the preoperative aspects of Transoral Robotic Surgery (TORS), including patient evaluation, instrumentation and set-up, moving on to diagnostic endoscopic and surgical techniques for tonsillectomy, laryngectomy, tongue base and glottic resection, and speech and swallowing considerations. They conclude with an examination of the potential for TORS in skull-base surgery, its future application and, importantly, telesurgery via the Internet. A pioneering book for a pioneering surgical technique in head and neck surgical technique and management, this should be on the shelf of every otolaryngology-head and neck surgeon.
